This page will be your "home
room" for online registration. After you have assembled all of data
from your company members, observers, etc.--using the "Registration
Worksheets" provided to you earlier online-- the next step is to fill out
the online forms, print them for your records, and submit them to the Long Beach Ballet.
You will need Acrobat Reader
installed on your computer. If you do not have it, you can download
it for free now.

There are five separate "form
categories" to be completed, in this order (links to the forms are at the
bottom of this page):
1 - Company and Director
Registration Form (1 page)
Your Company's travel plans and whether you wish to
perform at Disneyland.
Director registration including hotel room requests and
travel plans.
2 - Festival Package
Registration Form (1 page per hotel room)
This form is all participants (except Directors) who will be residing at the Festival Hotel (up to four persons per room).
3 - Day Observer and
Private Resident Festival Participant Form (1 page per person)
This form is for those who are not residing
at the Festival Hotel or who wish to participate in a portion of the
festival.
4 - Program and Technical
Information form (1 page per ballet)
Information for the souvenir program and for the
technical personnel.
5 - Summary Sheet (1 page)
A summary of your company's entire registration.
This form is to be printed and
returned to the Long Beach Ballet by
mail along with all participants' emergency release forms and
final payment by check.
Each form/page should be
filled out and saved to a new folder that you have created in your
computer (for example, you might create a new folder named "Festival
Forms"). You will need to provide a unique name to each page of the
same type so you don't overwrite previously saved forms. For
example, when you link to the Festival Package Registration Form
the first time and complete the information for the first hotel room, you
can save the form with its original name ("PackageRegForm").
However, when you have completed the form for the second hotel room, you
must give it a different name when you save it or you will erase what you
had saved before (you could call it "PackageRegForm2" for instance).
Now that all forms are in your
computer, you can easily check them for mistakes and make changes as you
wish. When you are confident that the forms are correct, you should
print each form for your records.
The next step is to attached
all the forms to an email and submit to
Registration@LongBeachBallet.com.
The data will be automatically compiled into spreadsheet format to
facilitate administration and scheduling.
And finally, mail the Summary
Form, a check for final payment, and the Release Forms to the Long Beach Ballet
at
1122 E. Wardlow Road, Long Beach, CA 90807
As many variables as possible have been
included on the forms. There might, however, be an exception that
was pre-approved by the Long Beach Ballet that is not on the form. These
can be explained on the final Summary Sheet.
