To register as an observer,
please select one of the forms below, fill it out, print it for your records, and
then submit it to the Long Beach Ballet by attaching it to an email
(instructions are on the form).
You will need Acrobat Reader
installed on your computer. If you do not have it, you can download
it for free now.
There are two
categories of Festival Observers: "Private Resident" and "Package"
(includes hotel). For detailed information about these categories,
click on the "Package" link at left.:
- Festival Package
Registration Form (1 page per hotel room)
This form is for Observers who will be residing at the Festival Hotel (up to four persons per room).
- Day Observer and
Private Resident Festival Participant Form (1 page per person)
This form is for those who are not residing
at the Festival Hotel -- one two or three days of the festival.
Each form/page should be
filled out and saved to a new folder that you have created in your
computer (for example, you might create a new folder named "RDAP
Forms"). You will need to provide a unique name to each page of the
same type so you don't overwrite previously saved forms. For
example, when you link to the Festival Package Registration Form
the first time and complete the information for the first hotel room, you
can save the form with its original name ("PackageRegForm").
However, when you have completed the form for the second hotel room, you
must give it a different name when you save it or you will erase what you
had saved before (you could call it "PackageRegForm2" for instance).
Now that all forms are in your
computer, you can easily check them for mistakes and make changes as you
wish. When you are confident that the forms are correct, you should
print each form for your records.
The next step is to attach
all the forms to an email and submit them to
Registration@LongBeachBallet.com.
